An experienced Patient Liaison Practitioner based at the Major Trauma Centre in South Wales will listen to what matters to you and then help you access the right information, guidance and support you need. If you are based at Trauma Unit, the Patient Liaison Practitioner will work with your Major Trauma Practitioner or Rehabilitation Co-ordinator to ensure that support is provided throughout the South Wales Trauma Network.
The approachable Patient Liaison Practitioners will meet with patients and families to offer guidance, reassurance and practical help when it is needed.
Support can also be accessed through this website, as well as via telephone and email, by speaking with the Major Trauma Support Service at Hugh James. Your Patient Liaison Practitioner, Major Trauma Practitioner or Rehabilitation Co-ordinator can guide you to our contact details. If you seek support via the website or by telephone or email, your query will be passed to our Major Trauma Support Service at Hugh James. Our approach:
Working together for better outcomes
We work closely with hospitals and support organisations across Wales to understand each person’s unique needs and we respond to the challenges they may face. We co-ordinate with healthcare professionals, specialist charities, local support services and legal experts to make sure you have access to the right help at every stage of your recovery, both during your hospital stay and once you return home.
This joined-up approach helps us provide timely support and create recovery pathways that feel personal to each patient and family meaning that you don’t have to navigate everything alone.
Our commitment
As part of our dedication to supporting recovery:
- A variety of support services are provided free of charge. Should you require legal advice, a free initial consultation will be provided to review and discuss your circumstances.
- Support is available to every patient and family who receive treatment from the South Wales Trauma Network.
- The focus is on holistic support and meaningful outcomes for patients and families.
We are committed to making a genuine difference to people’s lives. Our work is not just about providing legal services, but about supporting the whole recovery journey and helping people rebuild their lives in a way that respects their choices and circumstances.
If you or your loved one would like support, our Major Trauma Support Service team is here to help you access the guidance, connections and care you need.
